Why Fibromyalgia Hurts


Fibromyalgia pain:

  • Oxygen deficiency in the muscles; this stimulates a process which results in the breakdown of muscle proteins to amino acids that can be utilized to make ATP. This muscle tissue breakdown, which has been observed in muscle biopsies taken from fibromyalgia patients, is hypothesized to result in the muscle pain characteristic of fibromyalgia.
  • Abnormalities of increased deposits of glycogen (muscle sugar), abnormal mitochondria (organelles that produce the cellular fuel, ATP), DNA fragmentation, and damaged muscle cell membranes. Decreased levels of growth hormone, due to disrupted sleep, may be part of the reason for the unrepaired muscle damage seen in fibromyalgia.
  • High levels of sugar in the blood which causes a switch into glycolysis with production of lactic acid.It is this build up of lactic acid that causes the pain, heaviness, feeling exhaustion, deadened muscles and the sensation that muscles will not work, and it is this inefficient switch of energy production that causes the large build up of lactic acid which results in burning and pain. The lactic acid build up can also cause microscopic muscle tears which sets up an immune response of constant healing and repair leading to immune system depletion in fibromyalgia patients.
  • Fibromyalgia patients also have greater levels of substance P in their spinal fluid. This is one of the most definitive markers of fibromyalgia.
    Substance P is a brain chemical associated with pain, stress and anxiety, as well as depression.
    Low serotonin levels can cause elevated substance P levels and this may play a role in the pain associated with fibromyalgia as those with fibromyalgia are known to have low serotonin levels.
  • Mineral deficiencies and imbalances can also contribute to increased pain sensations in fibromyalgia patients.
  • Dehydration can also lead to increased pain. Fibromyalgia symptoms include dry mouth, skin and mucous membranes.

Systemic enzyme therapy is the use of enzymes to treat the entire system. These enzymes are coated or designed in such a way as to be protected from the stomach's acid. This allows them to travel to the intestine where they are absorbed into the bloodstream and carried throughout the body, helping to fight disease. Systemic enzyme therapy is effective in at least three ways:

1. Enzymes are able to penetrate the fibrin coating and break up the immune complexes (deposited in the tissues) into smaller sizes and to bring them back into the bloodstream for elimination. Effective Magnesium Regimens for Fatigue/Fibromyalgia

Eighty-six patients with chronic muscular complaints including myofascial pain, relapsing soft tissue injuries and fibromyalgia were treated with intramuscular or intravenous magnesium (Mg) in doses of 500-1,600 mg (Note: Presumably, this dose refers to the amount of Mg sulfate or Mg chloride, not the amount of elemental Mg used). Mg was given alone or in combination with B-vitamins, calcium, and either low dose (600-3,000 mg) or high dose (10-25 g) ascorbate. Of the acute cases, 74% improved; 53% of those who improved required only one injection and 32% required 2-4 injections for optimal results. Of the chronic patients, 74% improved; 64% of the cases required 4 or fewer injections for optimal results. A minority of patients required long-term oral or parenteral Mg to maintain their improvement. Side effects leading to discontinuation of therapy occurred in 4% of the chronic cases. Drug prescriptions for NSAIDs and muscle relaxants were virtually eliminated.

In a second study, the mean red-blood-cell Mg concentration was significantly lower in 20 patients with chronic fatigue syndrome (CFS) than in healthy controls. Thirty-two patients with CFS received intramuscular Mg sulfate (1 g) every week for 6 weeks or a placebo, in a randomized, double-blind trial. Twelve (80%) of the 15 patients treated with Mg reported improvement (more energy, better emotional state, and less pain) and fatigue was eliminated completely in 7 cases. Only 3 (18%) of 17 placebo-treated patients improved (p = 0.0015), and in none was fatigue completely eliminated. Red-blood-cell Mg returned to normal in all patients receiving Mg injections, but in only one patient given a placebo.

In a third study, 32 patients complaining of chronic fatigue received potassium magnesium aspartate (1 g, twice a day) orally for 4 weeks and a placebo for an additional 4 weeks, in a double-blind crossover trial. Twenty-one patients (66%) improved during the active-treatment period, compared with 3 (9%) during the placebo period.

Comment: There is evidence that impaired mitochondrial energy production is a factor in both chronic fatigue and fibromyalgia. This abnormality may be related, at least in part, either to Mg deficiency or to impaired intracellular uptake of Mg. Intramuscular or intravenous administration of Mg raises the serum concentration to a degree that is not achievable with oral administration. That might explain why parenteral administration of Mg appears to be more effective than oral Mg in the treatment of chronic fatigue and fibromyalgia. In my experience, approximately 50% of patients with chronic fatigue syndrome or fibromyalgia show considerable improvement after receiving several intravenous injections of a combination of Mg, calcium, B vitamins, and vitamin C (the so-called "Myers' cocktail"--see Gaby AR. Altern Med Rev 2002;7:389-403). This observation has been confirmed by Reed (see citation below) and others.

Earlier work demonstrated that an oral preparation, potassium magnesium aspartate, is also beneficial in the treatment of chronic fatigue. Aspartate is a purported mineral transporter, which is said to facilitate the entry of potassium and magnesium into the cells. I have found potassium magnesium aspartate to be an effective treatment for general fatigue, muscle spasms (including exercise-induced muscle cramps), and some cases of ventricular ectopic beats.

Why are vitamins and minerals so important in CFS/Fibromyalgia?

Dr. Janet Travell, White House physician for presidents John F. Kennedy and Lyndon B. Johnson and professor emeritus of Internal Medicine at George Washington University, co-wrote Myofascial Pain and Dysfunction: The Trigger Point Manual, which is acknowledged as the authoritative work on muscle problems. In one chapter alone, Dr. Travell and coauthor Dr. David Simons reference 317 studies showing that problems such as hormonal, vitamin, and mineral deficiencies can contribute to pain and muscle disorders.

Every vitamin and nutritional mineral is very important in some way to health. If the patient is low in any of a number of vitamins and minerals, their fibromyalgia simply will not subside.

Magnesium

Magnesium is involved in hundreds of different body functions, but is routinely low in the American diet as a result of food processing. The average American diet supplies less than 300 milligrams of magnesium per day, while the average Asian diet supplies over 600 milligrams per day. I generally recommend taking 200 milligrams of magnesium a day.  Adding calcium to the magnesium can lessen the diarrhea, but also decreases the magnesium's absorption.

If the patient is magnesium-deficient, their muscles will stay in spasm and the fibromyalgia will not resolve. This is but one of the reasons why taking magnesium is so critical. In addition, magnesium is important in energy production. Adding 900 milligrams of malic acid can dramatically help energy as well. Most magnesium is intracellular, making blood tests an unreliable measure. Keep in mind that magnesium blood tests do not drop below normal until severe magnesium depletion occurs and everyone with CFS/FMS, fatigue, or muscle achiness should take magnesium. In fact, as epidemiological evidence suggests that low magnesium intake is associated with increased heart attack deaths, I recommend magnesium supplementation for anyone on the standard Western diet. An exception is if kidney failure with a blood creatinine level over 1.6 milligrams per deciliter (mg/dL) is present.

Fibromyalgia – Information on Fibromyalgia

Fibromyalgia is a chronic condition characterized by widespread pain in your muscles, ligaments and tendons, as well as fatigue and multiple tender points places on your body where slight pressure causes pain. Fibromyalgia is more common in women than in men. Previously, fibromyalgia was known by other names such as fibrositis, chronic muscle pain syndrome, psychogenic rheumatism and tension myalgias. Fibromyalgia (FM) is a disorder classified by the presence of chronic widespread pain and tactile allodynia. While the criteria for such an entity have not yet been thoroughly developed, the recognition that fibromyalgia involves more than just pain has led to the frequent use of the term "fibromyalgia syndrome". It is not contagious, and recent studies suggest that people with fibromyalgia may be genetically predisposed. The disorder is not directly life-threatening. The degree of symptoms may vary greatly from day to day with periods of flares (severe worsening of symptoms) or remission; however, the disorder is generally perceived as non-progressive. Most patients with fibromyalgia say that they ache all over. Their muscles may feel like they were pulled or overworked. Sometimes fibromyalgia symptoms include muscle twitches and burning sensations. More women than men are afflicted with fibromyalgia, and it shows up in people of all ages. A conservative estimate of its prevalence is 2% of the general population, but it may be as high as 3-5%. No one knows what causes fibromyalgia. Anyone can get it, but it is most common in middle-aged women. People with rheumatoid arthritis and other autoimmune diseases are particularly likely to develop fibromyalgia. There is
no cure for fibromyalgia, but medicines can help you manage your symptoms. Getting enough sleep and exercising may also help. Fibromyalgia (FM) is a chronic pain disorder characterized by widespread musculoskeletal aches, pain and stiffness, soft tissue tenderness, general fatigue, and sleep disturbances. The most common sites of pain include the neck, back, shoulders, pelvic girdle, and hands, but any body part can be affected. Fibromyalgiapatients experience a range of symptoms of varying intensities that wax and wane over time. Taming the fibromyalgia dragon requires understanding of the many aspects of an individual’s symptom experience. Are your symptoms from your stomach, muscle? Do you have irritable bowel? Do you have fatigue? Do you have a substantially interrupted sleep pattern? Are you unable to do anything that you used to do due to unrelenting widespread pain? Fibromyalgia is agreed upon as one thing—a disease entity or a painful entity. In fact, fibromyalgia really is a calamity. It took years to be recognized. It was ignored and misunderstood, and there are still doubters who consider this disease a legend more than accepted disease entity. Fibromyalgia affects predominantly women (over 80 percent) between the ages of 35 and 55. Fibromyalgia can also affect men, children, and the elderly. It can occur independently, or can be associated with another disease, such as systemic lupus or rheumatoid arthritis. The prevalence of fibromyalgia varies in different countries. In Sweden and Britain, 1 percent of the population is affected by fibromyalgia. In the United States, approximately 2 percent of the population have fibromyalgia. Fibromyalgia is a syndrome rather than a disease. Unlike a disease, which is a medical condition with a specific cause or causes and recognizable signs and symptoms, a syndrome is a collection of signs, symptoms, and medical problems that tend to occur together but are not related to a specific, identifiable cause.

Fibromyalgia Facts:

  • Up to 80% of fibromyalgia patients suffer from IBS.

  • People with Fibromyalgia have been shown to have up to 50 times the normal level of tartaric acid in their blood. Tartaric acid is an organic acid that acts as a muscle toxin. It is more toxic than 100% pure alcohol (ethanol) which is lethal at about a 1 ounce dose. Tartaric acid can be lethal at half that dose. When yeast ferment sugar and turn it into ethanol, they also create tartaric acid. In animal experiments, it causes muscle damage. Fibromyalgia sufferers experience muscle and joint pain.

While the criteria for such an entity have not yet been thoroughly developed, the recognition that fibromyalgia involves more than just pain has led to the frequent use of the term "fibromyalgia syndrome". Not all affected persons experience all the symptoms associated with the greater syndrome.
